New Community Nursing Team launched in North Lincolnshire

A new nursing team has been set up to help people who don’t traditionally access healthcare services.

The Community Health Inclusion Nursing Team are working with local organisations and charities to help provide vital care to vulnerable people and those who don’t have an address… i.e the homeless, refugees, asylum seekers, those living in temporary accommodation and members of the travelling community.

The team, which includes district and community nurses, offer a wide range of services, from health and wellbeing advice and support to manage long term conditions, to foot and wound care, continence issues and injections/immunisations.

They’re holding regular drop-in clinics at community locations and are working with local charities to identify patients.
