NHS Diabetes Prevention Programme
The NDPP provides a unique opportunity to receive unbiased nutritional education, guidance on exercise, whilst also understanding the significance of other lifestyle factors such as sleep deprivation and stress on our health. We provide support over a nine month period to enable our patients to make small but sustainable changes, empowering them to maintain better health and quality of life for the rest of their lives.

To register via this route, patients will need:
Their NHS number
Latest HbA1c or FPG reading and date of reading within the last 24 months. (To be eligible for the programme via this route, the HbA1c reading would need to be between 42-47 mmol/mol or Fasting Plasma Glucose reading between 5.5- 6.9 mmol/l).
Their GP surgery name
Following submitting this information, our patient support team will be in touch.
The programme is also available via self-referral to any person who has previously been diagnosed with Gestational Diabetes – even if they now have a normal blood glucose reading. Sign up using this link: https://preventing-diabetes.co.uk/gestational-diabetes/. No recent blood test required if you have a history of Gestational Diabetes.
